WebbThe head impulse test may be positive (but it may also be positive for other peripheral causes of vertigo, and so cannot be used to differentiate between them). It is useful for helping to differentiate vestibular neuronitis from a central lesion. Hearing and otoscopy are normal on examination. WebbHI = Horizontal Head Impulse Test (h-HIT) The HIT is abnormal with vestibular neuritis (nerve issue, so reassuringly, not a central lesion).. Abnormal is good! (in someone with vertigo). HIT also normal in normal patient (so must only be interpreted in a patient who still has vertigo and nystagmus at the time of exam).
